This form will be used to verify your assets, your spouse's assets (if married), and your parents' assets (if a dependent student) on your FAFSA. If you are a dependent student, one parent must sign. Your application is incomplete until this document is received.

Important: Report amounts as of the date you signed and submitted your FAFSA — not today's date. If you were a dependent student, your parent(s) must include their assets. You cannot leave a space blank — enter "0" if the amount is zero ($0).

What was the net worth of your investments including real estate?
Do not include the family home. Net worth = market value minus debt. Includes rental property, trusts, mutual funds, CDs, stocks, 529 plans, etc.
What was the net worth of your businesses and investment farms?
Do not include the family farm you live on. Does not include a small business with 100 or fewer full-time employees.
